Source: DOL Form 5500 and Schedule A, plan year 2024, employer health plans with 100 or more participants, grouped by the sponsor's mailing city; built October 24, 2026. Methodology.
Which U.S. cities have the most employer health plans?
By employer mailing address on 2024 Form 5500 filings, New York, NY (1,465); Houston, TX (1,029); Chicago, IL (902); Atlanta, GA (612); San Francisco, CA (578) filed the most health plans with 100 or more participants.
Why is my city not listed?
A city gets its own page once at least 15 employers headquartered there filed a health plan with 100 or more participants for the latest plan year. Cities are taken from the sponsor's mailing address, so a plan administered from a parent company's office is counted there, and spelling variants of one city (St. Louis, Saint Louis) are combined; suburbs are never folded into the metro.
What does a city page show?
Each page lists the largest employer health plans in that city by premium, the brokers paid the most by those employers, the carriers writing the most medical premium, the median commission rate against the state and national medians, the funding split between fully insured and self-funded plans, and the change from the prior plan year.
